Our practice provides cardiology consult care and services to patients in skilled nursing facilities and other out-of-hospital facilities. We work alongside primary care and other providers to help care for patients with a cardiovascular history or need (heart failure, CAD, arrhythmia management, LVAD, Pacemaker/defibrillators, blood pressure, PAD, medication concerns, etc).  

 

We also offer Pulmonology consult care to serve patients with pulmonary needs (COPD, asthma, OSA, CPAP/BiPAP, tracheostomy, etc).

 

Our aim is to provide high quality care and optimize the care they receive at their facility in an effort to maximize their rehab potential and also reduce the number if incidences where patients need to return to the hospital. Our team of dedicated providers is here to care for patients and work in collaboration with providers and staff at various out-of-hospital facilities.
